Job Summary and Responsibilities CHI Saint Joseph Medical Group is looking to hire a Cardiology Advanced Practitioner to join our well established practice in Winchester, KY. This position is in an outpatient setting, working closely with our physicians to ensure great continuity of care for our patients. This position is full‑time with no mandatory call.

Responsibilities

Accurately assess patient conditions and document effectively to support charges and to communicate with other providers.

Provide comprehensive patient assessment, including functional, mental, social and physical conditions.

Establish accurate diagnosis.

Seek consultation/supervision of physician as needed to confirm proper assessment.

Appropriately involve patients and caregivers in treatment planning.

Provide interface between clinic and other providers to assure timely, comprehensive services for patients.

Independently develop and implement treatment plans and evaluate patient outcomes.

Take responsibility for clearly communicating patient needs, care plans and problem resolution to physicians and other healthcare teams.

Possess knowledge and skill to provide supervision to clinical staff as needed.

Develop/maintain policy and procedure for patient care activities.

Use process improvement skills to contribute to team development and program growth.

Essential Duties

Understand, support, and implement the mission of the facility to deliver health care to those in need in the performance of all job responsibilities.

Ensure that patient care meets or exceeds the facility quality standards.

Ensure that patient satisfaction meets or exceeds standards set by the facility. li>

Educate patients on both clinical and managed care.

Participate in clinic and network task forces as requested and appropriate.

Function as an effective clinic team member.

Meet productivity standards.

Effectively use clinic financial and other performance information.

Job Requirements

Candidate must be appropriately licensed as a PA or eligible for licensure in the Commonwealth of Kentucky.

Pay Range $42.49 – $60.25 per hour
