Responsibilities

Psychosocial Assessment – Performs psychosocial assessments, considering family, social, and medical history. Maintains current knowledge of medical and mental health diagnoses. Reports suspicions of abuse and neglect to legally mandated authorities. Completes documentation with accuracy and clarity.

Resource Support – Collaborates with the health care team to develop treatment plans and identify resources. Works with patients/families/caregivers to provide real‑time accurate information and condition‑specific resource information. Evaluates effectiveness of clinical interventions and makes changes as needed.

Psychosocial Interventions – Provides psychosocial services per the patient’s comprehensive plan of care. Offers supportive counseling to improve emotional coping and adherence. Educates patients/families/caregivers and health care team on treatment options. Provides self‑management support to high‑risk/complex patients. Offers groups and programs for special populations. Gives anticipatory guidance to reduce discharge barriers. Coordinates family‑centered care team meetings. Provides crisis assessment and interventions, collaborates with referral sources.

Interdisciplinary Collaboration – Works to improve system and promote well‑being of patients/families. Acts as liaison between patient/family/caregiver and medical team and outside agencies. Mediates within the medical team. Empowers caregivers to problem‑solve and pursue alternative plans. Facilitates communication and coordination while involving patients/families in decision making.

Collaboration – Partners with patient, family, and healthcare team for safe transition of care. Educates professionals or students about psychosocial aspects of care. Promotes a positive work environment. Participates in professional growth and attends meetings, conferences, and in‑service programs.

Qualifications

Master’s degree in Clinical Counseling or Social Work

2+ years of work experience in a related discipline

LPCC or LISW license in Ohio

May require clinical license, certification, or training
