Responsibilities

Patient Care – Age Specific and Culturally Diverse. Consistently integrates age‑specific and culturally diverse concepts into patient care, taking into consideration both the patient's chronological age and developmental functioning.

Interdisciplinary Collaboration. Works for system improvement, promoting patient/family/caregiver well‑being. Acts as a liaison between the patient/family/caregiver, the medical team and outside agencies, mediates within the team, and facilitates communication and coordination while involving the patient/family/caregiver in decision‑making to minimize service fragmentation.

Psychosocial Assessment. Conducts comprehensive psychosocial assessments of the patient/family’s health‑care needs, identifies strengths, challenges and opportunities, evaluates safety concerns, intervenes to reduce risk, reports suspected abuse or neglect to mandated authorities, and uses the assessment to establish treatment interventions in the comprehensive plan of care.

Psychosocial Interventions. Provides psychosocial services as identified in the patient’s comprehensive plan of care, supportive counseling to improve emotional coping and adherence, collaborates with referral sources and health‑care team members, educates patients/families/caregivers and team members about treatment options, offers self‑management support for high‑risk/complex patients/families, provides group and program services for special populations, and furnishes crisis assessment and interventions to restore equilibrium.

Resource Development and Utilization. Develops resource networks, utilizes resources effectively, serves as a resource to peers, identifies needs, develops programs, evaluates outcomes for special populations, and implements changes based on outcomes. Facilitates collaboration with community agencies to enhance intervention processes and advocates for patient populations on a systems level.

Leadership. Serves as a recognized expert in social‑work practice, providing work direction, leadership, mentoring, coaching, recruitment participation, scheduling coordination, clinical activity coordination, performance evaluation input, goal development, and liaison activities with school districts and the division.

Compliance. Ensures that the clinical practice of all staff conforms to local, state, and federal regulations and accrediting standards, maintains clinical competency, responds to and documents complaints, meets continuing education requirements, complies with specific procedures for infants, children, and adolescents, and adheres to Cincinnati Children’s policies and procedures protecting patient rights and confidentiality. Maintains timely, accurate, and secure information, attends required training sessions, and complies with infection control policies.

Qualifications

Master’s degree in Social Work or Counseling

5+ years of work experience in a related discipline

Current LISW or LPCC license to practice in the State of Ohio

Must reside in Portsmouth, OH and surrounding area only

Location South Campus – Portsmouth, Ohio and surrounding areas

Schedule & Shift Full time, Day (United States of America)

Pay Range Annualized pay may vary based on FTE status $81,723.20 – $104,208.00
