Overview
The Care Manager/“Designated Care Manager” is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe and comfortable home-like environment. The Care Manager demonstrates the Sunrise Mission, “to champion quality of life for all seniors” in accordance with applicable laws and Sunrise standards, policies, and procedures to promote the highest degree of quality care and services to residents. This range is provided by Sunrise Senior Living. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more. Base pay range $17.20/hr - $21.55/hr Sunrise Senior Living offers benefits and compensation that include medical, dental, vision, life, disability plans, retirement savings, PTO, sick time, holidays, tuition reimbursement, and other programs. Resident Care Review, read, notate, and initial Daily Log to document resident information. Participate in the development of ISPs and monthly updates. Review designated assignments. Be responsible for a designated group of residents during the shift, knowing resident locations and performing checks. Observe, report, and document changes in residents’ condition (skin, behavior, alertness, weight, diet, activity participation). Attend daily Cross Over meetings with the lead care manager. Notify supervisor and resident care director if a resident’s care needs increase. Inform supervisor of resident changes in condition. Respond to security system and resident call bells promptly; take appropriate action including resetting bells. Greet guests, family members, residents, and team members; answer calls professionally. Assist with continence management and sanitation of resident suites and restrooms per standard procedures. Communicate with families as needed and serve as a resource. Help residents maintain independence and safety while upholding Resident Rights and Sunrise Principles of Service. Participate in ADLs and Invite, Encourage, and Assist (IEA) residents to participate per ISP. Engage residents in life skills and enrichment activities, including reminiscence. Demonstrate empathy and adapt to each resident’s communication style and needs. Maintain a warm, respectful, and calm environment; follow safety regulations and infection control practices. Risk Management and Safety Partner with the community team to ensure compliance with health and safety regulations; promote risk management programs. Practice safety procedures including PPE, fire safety, SDS, and lockout/tagout as applicable. Report all accidents/incidents and unsafe conditions immediately. Keep hazards and equipment out of walkways and exits; follow fire codes. Follow proper procedures for exposure to bloodborne pathogens and hazardous materials. Ensure oxygen tanks are stored safely and monitored. Housekeeping, Laundry, and Dining Maintain and clean resident rooms and common areas; perform house-keeping tasks as assigned. Wash resident laundry as ISP requires; manage dining room linens and napkins. Serve meals in the dining room and assist with setup/cleanup as assigned; follow dietary guidelines per ISP. Observe, document, and respond to dining habit changes; provide room service as needed. Reminiscence Neighborhood Care IEA residents in life skills and enrichment activities per ISP and demographic profile. Encourage attendance at afternoon social; incorporate Creating Pleasant Days into routines. Integrate life skills into daily routines and blend multisensory experiences. Participate in monthly letters home with residents and families. Team Training and Core Competencies Collaborate with the team and contribute to engagement programs and improvement planning. Attend regular meetings and ongoing training; comply with infection control policies. Maintain required trainings and regulatory compliance. Perform other duties as assigned. Core Competencies Ability to stay calm in difficult situations and manage multiple priorities. Strong written and verbal communication; strong organizational and time management skills. Good judgment, problem solving, and decision-making abilities. Experience and Qualifications High School diploma/GED; may be required by state/provincial regulations. CPR and First Aid as required by regulations. Must be at least 18 years old. Previous experience with seniors preferred; desire to serve and care for seniors.
