Industry Hospital & Health Care Type Privately Held, VC-backed Founded 2021 Employees 51-200 Funding $51-$75 million About the Role The Company is actively seeking a Vice President of Patient Care Navigation to spearhead the integration of new partnerships, focusing on data-driven, proactive clinical interventions and ensuring a compassionate patient experience. In this pivotal role, you will manage the complete implementation lifecycle, from pre-signature planning through to post-signature execution, while collaborating with partners to enhance our Model of Care. You will be responsible for designing and refining patient-centric clinical programs, establishing evidence-based workflows, and implementing scalable care navigation strategies. Additionally, you will inspire and lead high-performing clinical and non-clinical teams, ensure regulatory compliance, and drive the adoption of AI and automation tools to enhance clinical decision support and care coordination. Qualifications A minimum of 10 years of progressive leadership experience in clinical operations or related healthcare domains. Proven track record in mid-to-senior management roles. A Master's degree in a relevant field is required, with an advanced clinical degree preferred. Deep understanding of care coordination requirements across various healthcare programs. Clinical proficiency in managing mental health and chronic conditions. Experience in designing and scaling patient-centric clinical programs. Competence in process improvement, problem-solving, and integrating technology-enabled care delivery models. Strong background in care management operations within Medicaid, Medicare Advantage, or ACO environments. Ability to lead quality assurance initiatives and maintain transparent communication with stakeholders.
