Join our dedicated primary care team in Cheshire, CT, where small‑town charm meets modern healthcare. Hartford HealthCare is seeking a compassionate Nurse Practitioner or Physician Assistant for our adult medicine outpatient practice.
Location: 280 South Main St, Cheshire (10471)
Shift: Monday‑Friday, weekdays only. Full‑time. No call required.

Scope of Practice Under established guidelines and in collaboration and/or supervision with the practice physician(s), NPs and PAs perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.
In consultation with the physician(s) and care team members, NPs and PAs develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
NPs or PAs provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identify problems outside of their scope of practice and refer appropriately to physician.
Qualifications
Licensure: Unrestricted State of Connecticut Medical License (or eligible)
Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA
Experience: 1+ years of experience in a primary care or related outpatient setting
