Job Overview Community of Hope is seeking a Training and Outreach Specialist for the HONEY program, a family health and birth center in Northeast Washington, DC. The role educates, engages, and builds relationships with individuals and families who are experiencing homelessness and are pregnant, connecting them with appropriate support systems and community resources.
Responsibilities
During initial intake, assess client connections or barriers to receiving perinatal services, preferences for care, risk factors, and work to opt‑in client to HONEY perinatal care coordination.
Identify and link participants to appropriate community resources, perinatal care, and wrap‑around supports in coordination with the assigned housing case manager.
Opt‑in, schedule, and send rides for patients through the Rides for Moms partnership with Uber Health (approximately 30 rides per month).
Document relevant patient encounters in eCW within 3 business days and update referral and client tracking databases within 1 business day of interaction.
Conduct outreach to develop relationships with HONEY program participants and partner with government and community organizations.
Build partnerships with homeless service and health‑care providers to bring referrals, programming updates, and additional support (e.g., training).
Attend community events, tabling events, meetings, and conferences to promote the HONEY program to potential clients and partners.
Educate partners, clinicians and housing system staff on perinatal health issues to support families during pregnancy and infancy.
Deliver program overviews and educational sessions virtually, in person, and asynchronously to health‑care and homeless service providers (approximately 2‑4 sessions per month).
Consult with the COH Maternal & Child Health and Housing teams to develop presentations and training materials.
Must‑Haves
Bachelor’s degree
Prior instructional experience
Strong organizational, written, and verbal communication skills
Strong interpersonal skills and comfort in conducting cold calls and outreach
Basic graphic design, marketing, and presentation skills
Reliable transportation
Nice‑to‑Haves
Experience in adult education, social services, health care, or nursing
Experience working with under‑resourced populations, including pregnant people experiencing homelessness
