Join Wellspring as an ACT Targeted Case Manager (TCM). Wellspring has championed mental health recovery in Louisville since 1982, supporting adults with psychiatric disabilities through behavioral health, housing, and employment services.
Company Overview Wellspring provides recovery‑oriented programming utilizing a multi‑disciplinary, person‑centered approach. We focus on housing, medical care, behavioral health, and community integration for people with serious mental illness and chronic homelessness.
Responsibilities
Assist clients in accessing medical, social, educational, and other support services.
Develop and facilitate individualized recovery goals via assessment, connecting to resources, and monitoring progress.
Travel throughout the community conducting home visits and connecting clients with resources and services.
Collaborate with landlords and community partners, advocating for client needs and rights.
Help clients locate, apply for, and move into housing.
Provide illness management, medication management, and independent living skills, including crisis intervention when needed.
Engage clients in physical, mental health, and addiction treatment to ensure continuous care.
Participate in weekly, monthly, quarterly staff meetings, on‑call rotations, and daily collaboration with Wellspring clinical staff and other providers.
Qualifications
Personal vehicle required for traveling between work sites and client appointments.
KY TCM Training.
BA/BS in psychology, social work, or related field recognized by DBHDID, with at least one year of full‑time experience with adults with serious mental illness OR a related master’s degree.
One year of experience working with adults with substance use disorders is preferred.
Salary & Benefits
$42,000 – $50,000.
Compensation for on‑call rotations.
Mileage reimbursement for work‑related travel.
