Per Diem Speech-Language Pathologist (SLP)
Location: Prestige Care, Fairfield, Iowa
Department: In-House Subacute Rehabilitation
Schedule: Per Diem / As Needed

Position Summary
Prestige Care – Fairfield is seeking a Per Diem Speech-Language Pathologist (SLP) to join our in-house subacute rehabilitation team. The SLP will evaluate, plan, and provide treatment for residents and patients with speech, language, cognitive, voice, and swallowing disorders. This role supports the rehabilitation team in promoting optimal communication and swallowing function to enhance quality of life and successful transitions of care.
Key Responsibilities
Conduct speech, language, cognitive-communication, and swallowing evaluations for patients in a subacute rehabilitation setting.
Develop and implement individualized treatment plans based on patient needs and goals.
Provide direct therapy services in accordance with physician orders, regulatory standards, and facility policies.
Document evaluations, progress notes, and discharge summaries accurately and timely in the electronic medical record.
Educate patients, families, and caregivers on communication and swallowing strategies, home programs, and safety precautions.
Collaborate with an interdisciplinary team (PT, OT, nursing, physicians, and social services) to ensure coordinated care.
Participate in case conferences, care planning meetings, and ongoing clinical training.
Maintain compliance with all state and federal regulations, as well as facility policies and procedures.
Qualifications
Required: Master’s degree in Speech-Language Pathology from an accredited program.
Required: Current Iowa Speech-Language Pathologist license (or eligibility).
Required: Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA.
Preferred: Prior experience in subacute, skilled nursing, or long-term care settings.
Excellent communication, clinical reasoning, and time management skills.
