Responsibilities: Manage a caseload and complete member assessments in home, community, or telephonic settings. Provide clinical support and collaborate with the Interdisciplinary Care Team (ICT) for members with medical complexity. Develop, implement, and coordinate individualized care plans to ensure appropriate services and address barriers to care. Conduct medical attestations, clinical reviews, and ensure services are provided in the least restrictive setting. Support members and caregivers in accessing social, housing, educational, and other community resources. Serve as the primary point of contact for the ICT and coordinate with internal and external stakeholders to meet member needs.
What You’ll Bring: Michigan LBSW or LLBSW (MSW preferred) 2+ years in behavioral health or clinical care management Excellent communication and coordination skills Valid driver’s license, reliable car, and home office with internet
