Position Summary Join our passionate and skilled team as a Clinical Pharmacist Specialist in Hematology, Oncology, and Bone Marrow Transplant services. In this dynamic role, you will rotate between inpatient and outpatient service lines, providing comprehensive pharmaceutical care throughout the continuum of cancer treatment. You'll have the unique opportunity to collaborate with information technology colleagues to develop chemotherapy Computerized Provider Order Entry (CPOE) protocols. Responsibilities Foster interdepartmental relationships to enhance communication regarding clinical pharmacy services and patient care across the organization. Deliver patient- and family-centered education and support during transitions of care. Engage in medication reconciliation, addressing and resolving medication order issues with prescribers. Design and implement guidelines and interventions for antimicrobial use based on continuous review and analysis of daily audits. Assess and manage medication orders for therapeutic appropriateness, considering age, weight, laboratory values, diagnosis, and relevant clinical data. Monitor for clinical alerts (allergies, interactions, therapeutic duplications, ADRs) and implement interventions to achieve both cost containment and evidence-based goals. Communicate effectively with interdisciplinary teams to ensure continuity of care and respond promptly to all modes of communication. Participate in patient safety committees, quality improvement initiatives, and manage drug shortages effectively. Educate pharmacy staff, physicians, nurses, and other healthcare professionals through in-services and lecture series. Act as a clinical preceptor for pharmacy students, residents, and other healthcare trainees. Qualifications Education:

Doctorate (Pharm.D.) in Pharmacy or equivalent. Experience:

At least 3 years of pediatric pharmacy practice in an acute care setting or completion of a PGY-1/PGY-2 residency in pediatrics, with experience in medication products, pharmacokinetics, disease states, and therapeutic drug monitoring. Licensure & Certifications:

Current licensure as a pharmacist in Arizona (or reciprocal) and BLS, ACLS, or PALS certification. Board certification in Pediatrics or Pharmacotherapy is preferred. Skills:

Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook) and pharmacy clinical information systems; strong communication, analytical, and interdisciplinary collaboration skills; capability to precept students and residents. Physical Requirements:

Ability to lift up to 35 lbs occasionally; routine standing, walking, sitting, with occasional pushing/pulling; exposure to bloodborne pathogens and hazardous materials in a clinical setting. Phoenix Children's Mission, Vision & Values Mission:
