Clinical Pharmacist - Evening ICU (7 on/7 off)

Position Summary We are seeking an experienced Inpatient Clinical Pharmacist to join the Thomas Campus team, specializing in pediatric critical care. The role features a 7 on/7 off schedule with 10‑hour shifts. Preferred qualifications include pediatric and critical care pharmacy experience and completion of an ASHP‑accredited PGY‑1 residency. We offer relocation assistance and a starting bonus up to $10,000.
Position Duties
Reviews and manages medication orders for therapeutic appropriateness based on age, weight, height, laboratory values, diagnosis, and other patient and clinical information, and intervenes as necessary.
Effectively screens for and manages clinical alerts such as allergies, interactions, therapeutic duplications, and adverse drug reactions.
Actively problem‑solves medication orders and contacts prescribers to recommend appropriate alternative therapies.
Complies with Pharmacy and Therapeutics Committee directives related to formulary management.
Documents clinical interventions to meet or exceed established cost containment and evidence‑based medication therapy goals.
Prepares and dispenses sterile and non‑sterile medications in compliance with policies, goals, and laws.
Demonstrates appropriate clinical pharmacy skills, including assessing and formulating medication therapy recommendations that are safe, evidence‑based, and cost‑effective.
Participates in decentralized pharmacist‑assigned duties and assists in developing guidelines and policies for medication use.
Provides ongoing education and training to pharmacists, residents, interns, technicians, and student learners, and orients new employees.
Directly supervises technical and support staff to ensure accuracy in medication preparation and dispensing.
Acts as liaison between pharmacy and medical/nursing staff, participating in hospital and department operational and quality improvement initiatives.
Serves on unit‑based medical and clinical teams, providing information and recommendations for service and patient care.
Supports antimicrobial stewardship program activities and other advanced clinical services.
Provides support for hospital quality and safety teams.
Effectively communicates problems and patient needs to incoming staff to enhance continuity of care.
Responds promptly to all modes of communication within the department and hospital.
Initiates and completes incident, adverse drug reaction, and controlled substance documentation reports.
Participates in drug use evaluation, quality assurance, and performance improvement activities.
Maintains investigational drug study records and documentation as required.
Provides evidence‑based medication education to patients, caregivers, and staff for inpatient and outpatient areas.
Calibrates medication counseling and home medication screening.
Performs miscellaneous job‑related duties as requested.
Qualifications
Education
Bachelor’s Degree (BS) or Doctorate Degree (Pharm.D.) in Pharmacy (Required)
Completion of PGY‑1 pharmacy residency accredited by ASHP (Preferred)
Experience
Experience with information systems, automated, and technical equipment (Required)
One year of experience in a hospital pharmacy practice or equivalent (Required)
One year of previous pediatric patient population experience or equivalent (Preferred)
Special Skills
Experience with computer software, including MS Office Suite (Required)
Ability to problem‑solve and collaborate, demonstrating empathy, teamwork, and constructive communication (Required)
Additional Requirements
Current licensure as a pharmacist (Required)
Current BLS and PALS or ACLS from American Heart Association (Required)
