Independent Optometrist – Walmart
Walmart seeks an Independent Optometrist to lease space in our stores and provide eye and vision care. You will assume a leading role in patient care, managing your own schedule and fees while maintaining high standards of service.
What You’ll Do
Launch your services in Walmart stores.
Take a leading role in patient care and general health and well‑being.
Establish your days and hours of practice.
Control your patient schedule and set your fee schedule.
Keep 100 % of exam/professional fees.
Optionally procure equipment/instrumentation and administrative/Opt-Tech support services if state allows.
Minimum Qualifications for Lease Agreement
Doctor of Optometry (OD) from an accredited optometry school.
Valid state optometry license.
Ophthalmologist (MD or DO) from an accredited school.
Valid state ophthalmology license.
General Liability Certificate.
Professional Liability Insurance Certificate.
W‑9.
