RN Care Manager – Bilingual – Cantonese/Mandarin
VillageCare , a community‑based, not‑for‑profit organization serving people with chronic care needs in New York City, is seeking a full‑time RN Care Manager on its DSNP team. The role requires bilingual proficiency in Cantonese and/or Mandarin and provides an opportunity to make a meaningful impact in the lives of our diverse members.
This fully remote position allows you to work from home while collaborating with a dynamic, customer‑focused team dedicated to excellence in healthcare.

Responsibilities
Conduct ongoing assessments of members’ health, environmental, and psychosocial status using NYS assessment tools.
Develop and coordinate comprehensive care plans through telephonic contact and other modalities.
Ensure each member receives tailored interventions addressing their unique clinical, psychosocial, and financial needs.
Oversee quality and effectiveness of services while facilitating smooth transitions from hospitals or nursing homes.
Collaborate closely with the Interdisciplinary Care Team, continuously monitoring and adjusting care plans to maximize health and functional capabilities.
Maintain compliance with local, state, and federal regulations.
Requirements
Valid NYS Registered Nurse license and registration.
Fluency in Cantonese and/or Mandarin.
Proficiency in care‑management software and Microsoft Office.
Strong analytical skills for accurate assessment of members’ health and psychosocial needs.
Collaborative ability with Interdisciplinary Care Team.
