Pediatric Nurse Practitioner Opening in California
Location: Madera, CA
Position Summary A dynamic community health center is seeking a Pediatric Nurse Practitioner (PNP) to provide comprehensive, compassionate care to pediatric patients. The PNP will perform advanced health assessments, manage acute and chronic pediatric conditions, and deliver preventative care in a collaborative team-based clinical environment. Key Responsibilities Clinical Care
Provide health maintenance care including well‑child exams and immunizations
Conduct routine developmental screenings and treat common childhood illnesses
Complete thorough documentation using SOAP format and Electronic Health Records (EHR)
Order diagnostic tests and interpret results; manage acute and chronic conditions
Coordinate follow‑up care and ensure continuity of care for chronic conditions
Participate in the on‑call rotation (approximately every 3–5 months for weekend or evening phone triage)
Refer patients to other healthcare providers or specialists per approved protocols
Medical Records
Timely and accurate charting for each patient encounter
Comply with clinical documentation and record‑keeping standards
Review and sign off on medical records, lab reports, and referral documents
Consultation & Referral
Collaborate with the Medical Director and physicians for complex cases
Coordinate referrals for specialty care, health education, or additional clinical support
Team Collaboration & Continuous Improvement
Participate in peer reviews, quality improvement (CQI) initiatives, and staff meetings
Attend monthly general and clinician staff meetings
Engage in daily team huddles to ensure high‑quality, individualized patient care
Provide insight on clinical resource needs and improvements
Minimum Qualifications Education
Graduate of an accredited Nurse Practitioner Program
Licensure & Certification
Valid California Nurse Practitioner License
Current CPR/BLS certification
Preferred Skills & Attributes
Bilingual (English/Spanish) strongly preferred
Strong interest in pediatric preventative care and chronic disease management
Excellent interpersonal communication skills
Demonstrates alignment with the mission of community‑based healthcare
Physical Requirements
Ability to move up to 20 lbs and push up to 50 lbs (on wheels)
Sufficient hearing and vision for clinical functions and documentation
High manual dexterity and physical mobility including bending, reaching, standing, kneeling
