As a member of the Opioid Response team, the Opioid response Linkage Navigator role will support CrescentCare’s community based navigation and opioid response efforts. The Prevention Department at CrescentCare supports several opioid response programs, including the New Orleans Syringe Access Program and the new Opioid response Mobile Unit. The Opioid response teams work to link key populations to opioid response services and care, and coordinates with community partners to implement opioid response programs across the New Orleans Region. The Navigator will conduct counseling and testing sessions, serve as navigation and linkage for patients accessing the mobile unit, support CrescentCare’s opioid response programs and community of practice, and support adherence/retention of clients in HIV/STI/Hepatitis C (HCV) services through linkage and referrals into other Prevention programming and agency wide services. Work with the Prevention Director, Prevention Linkage team, and other agency departments and partners, including medical providers, to actively test, link, and navigate clients to services including medical care and essential support services. Assist in compliance with funder requirements and attainment of program and outreach deliverables. Assist supervisor and managers with meeting program area requirements, including data management, analysis and reporting, staff and staffing needs, supplies, and all quality assurance (QA) measures. Ensure all reporting and evaluation mechanisms are complied with. Have a working knowledge of opioid response policy. Key Duties And Responsibilities
Liaise with Prevention team members, Call Center staff, medical providers, external stakeholders, and Director of Prevention to ensure newly identified patients are linked to care. Provide client-centered navigation and support services to priority populations accessing opioid response services. Conduct HIV/HCV/STI counseling and testing Collaborate with other departments and organizations to coordinate services to ensure client needs are met. Assist in data collection and data analysis, including electronic medical record follow up, to measure achievement of linkage deliverables for prevention grants. Conduct support for adherence and retention to HIV care services, ART treatment, HCV treatment, STI treatment, and PrEP services. Track all encounters with patients receiving services. Organize and conduct linkage in the context of street outreach, community outreach, testing, and educational events, and other locations as identified by project leadership. Assist with sexual health and opioid response community education curriculum and marketing initiatives. Work with community organizations to build and foster opioid response linkage programs. Help teach and present for organizations on sexual health, opioid response, and HCV prevention. Assist with monitoring and evaluation. Work with other appropriate staff to implement OPH requirements on care and social service referrals and Pre-Exposure Prophylaxis (PrEP). Other duties as assigned. Qualifications
High School Diploma/GED and 5 years' work experience with priority populations at high risk for HIV and Hepatitis C, required or Bachelor's degree and competency with Excel/data entry/computer literacy. Comparable education and experience will be considered. Skills & Competencies
Ability to work in different environments, including community venues and outdoors in differing weather conditions. Ability to work during non-traditional hours and on weekends. Flexible and non-judgmental ability to work with stigmatized populations and the general population. Culturally open when interacting with populations/clients of different backgrounds and value systems. Necessary technical skills to use electronic health record, incident reporting system, email, and other information systems. Ability to relate to all persons, especially key populations including Black Men who have sex with Men. (MSM) Latino MSM, MSM, Transgender Individuals, Injection Drug Users, and High-Risk Heterosexuals. Excellent verbal and written communication skills Ability to take initiative, problem-solve and follow through on tasks and duties to completion. Able to multi-task and function in an office with high call volume and face to face patient encounters Possess telephone etiquette skills to provide customer service for all patients. Able to manage time effectively, report to shifts on time, and complete full work shifts. Ability to return communication in a timely manner including same day as necessary. Willingness to provide services at community locations. Ability to complete tasks in a timely manner. Willingness to pick up testing shifts. Spanish Language Skills (Preferred) Essential Physical Requirements
Be able to stand for extended periods of time (3-4 hours) for outreach and community events. The ability to lift and carry 25-pound boxes (condom and counseling supplies) for outreach events. Ability to climb stairs to do in community venues and social events in urban locations with limited or no ADA compliance services in office-based and community-based settings where climbing several flights of stairs might be necessary. May need to lift up to 25 pounds in equipment and supplies. Total Compensation Package
