Responsibilities Work in an acute inpatient case management department Patient population includes diverse acute care inpatients requiring discharge planning and care coordination CarePort EMR is required; experience with AllScripts and GEMS/Sunrise is preferred 4x10 hour shifts from 7:30am to 6pm with required weekend rotation Travelers may float to a nearby sister hospital as needed Responsible for effective case management including facilitation of payor authorization processes, collaborative patient care management, care facilitation, and discharge planning Accountable for a designated patient caseload, aiming to manage length of stay and promote efficient resource utilization Provides clinical documentation support and acts as a resource for coders to ensure accurate patient care records Online modules must be completed and passed prior to start, typically one week before assignment No blocked scheduling is allowed Holiday and weekend coverage may be required as part of unit rotation Time off must be requested prior to assignment and is subject to approval after offer Allow a minimum of two weeks for background check and drug screen results Requirements Active and valid MI RN license required Bachelor of Science in Nursing (BSN) required AHA Basic Life Support (BLS) certification required Minimum 5 years of acute inpatient case management experience with strong acute discharge planning skills required Minimum 2 years of experience in the role required Previous experience charting in CarePort required; AllScripts and GEMS/Sunrise preferred Strong analytical, data management, PC, organizational, interpersonal, and negotiation skills required COVID vaccination is mandatory Benefits High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts American Traveler Job ID P-631288.
