Case Management About the Role
We are seeking an experienced Registered Nurse Case Manager to support acute and inpatient utilization management at a leading healthcare facility in Sacramento, CA. This role involves coordinating care across the continuum, ensuring medical necessity, and facilitating efficient resource use for patients with complex health needs. The ideal candidate brings strong clinical judgment, familiarity with regulatory guidelines, and a collaborative approach to care planning. Responsibilities
Conduct preauthorization, concurrent, and retrospective utilization reviews for ambulatory, acute, and post-acute patients Apply accepted criteria (Medicare, MCG, InterQual, health plan policies) to confirm medical necessity and level of care Manage inpatient and ambulatory referrals, including prior authorizations and retrospective reviews Collaborate with UM Medical Directors on cases not meeting criteria Issue and document authorization and denial letters in EPIC and Midas systems Participate in interdisciplinary care planning meetings with physicians, nurses, social workers, and rehab teams Coordinate discharge planning and post-acute referrals using community resources Facilitate repatriation of out-of-network patients and escalate delays as needed Refer patients to Value-Based Case Management programs for complex care needs Ensure compliance with delegated health plan contracts and regulatory timeframes Understand scope of authorization, financial responsibility, carve-outs, and risk-sharing arrangements Requirements
Active California RN license Minimum 4 years of acute/inpatient case management experience BLS certification Strong working knowledge of Medicare, MCG, InterQual, and health plan criteria Experience with EPIC and Midas documentation systems Familiarity with delegated health plan operations and contract implementation Additional Details
