Clinical Pharmacist
– Yakima Valley Farm Workers Clinic, Salem, OR
Overview Join our team as a Clinical Pharmacist and be part of a community health organization dedicated to improving access to quality care in underserved areas. We are one of the region’s leading community health centers, offering a comprehensive range of medical, dental, pharmacy, behavioral health, and supportive services.
What You’ll Do
Provide patient‑centered pharmacy services through in‑person or virtual appointments.
Support coordinated care at local clinics, including disease‑state and population health management.
Conduct medication therapy management (MTM) appointments and educate patients on self‑management.
Sit on local or corporate committees (e.g., Care Measurement, Med Management, P&T).
Perform drug‑utilization review, patient counseling, drug‑regimen review, and medication therapy management.
Collaborate on drug‑therapy management or other post‑diagnostic disease‑state management per a valid agreement.
Prescribe drugs or devices as authorized by statute according to established protocols.
Order, interpret, and monitor laboratory tests.
Receive or transfer prescriptions.
Oversee pharmacy personnel and ensure adherence to scope of practice and licensure.
Maintain pharmacy security and safeguard against theft or diversion.
Ensure accurate record and inventory maintenance per state and federal regulations.
Distribute prescriptions and drug orders correctly per provider authorization and YVFWC policies.
Stay current with pharmacological knowledge through workshops, publications, networks, and professional societies.
Perform clinical duties in alignment with TJC, HRSA, and YVFWC requirements.
Complete additional duties as assigned.
Qualifications
Bachelor’s Degree in Pharmacy.
Minimum 3 years of experience as a licensed Pharmacist, OR PGY‑1 residency + 1 year of Clinical Pharmacist experience.
Must hold an active Pharmacist License.
Ability to travel to surrounding YVFWC sites for coverage.
Drug‑testing required for employment and ongoing testing.
