Job Summary The Case Manager in the Youth Transitions Project (YTP) is directly responsible for service provision and housing assistance for participants in YTP. This program serves youth aged 18‑25 and youth heads of households aged 18‑25 in a rapid re‑housing model who are literally homeless. Service modalities include Housing First, Trauma‑Informed Care, Harm Reduction, Motivational Interviewing, and Critical Time Intervention. The role will work closely with the Youth team at Bannock Youth & Family Center, conducting needs‑based assessments, community outreach, case management, housing navigation, and coordination with service providers.

Essential Job Duties

Ensure that the youth’s voice is heard and that youth ideas shape programming.

Possess in‑depth knowledge of program requirements, referral processes, support services, HUD rapid re‑housing standards, and the coordinated entry system.

Provide intensive case management, ensuring regular engagement in services for the acute youth population.

Collect and maintain necessary documentation for housing, identification, income changes, and household changes.

Identify and serve the unique needs of participant households using screening and assessment tools.

Maintain service delivery database systems, attend intakes, housing orientations, lease‑ups, and educational opportunities for participants.

Assists clients in searching for housing, completing rental applications, interpreting leases, and understanding tenant rights.

Facilitate delivery of appropriate services for youth participants through an equity lens.

Provide housing navigation and process referrals as needed, including during vacancies.

Develop and monitor quality Housing Focused/CTI‑based supportive services tied to housing stability.

Provide tenant/landlord mediation and respond to landlord requests within two business days.

Attend orientations, trainings, and conferences as requested, and may provide mentorship to team members.

Document all client interactions and maintain confidential information in accordance with the Social Work Code of Ethics.

Perform all other duties as assigned.

Position Salary Range $21.00 – $23.00 per hour, based on experience.

Minimum Qualifications

Bachelor’s degree in human services, social work, or a closely related field, or related experience.

At least one full year of work with the homeless population.

Colorado driver’s license and state‑mandated automobile insurance.

Preferred Qualifications

Familiarity with current housing market information, housing trends, and available resources.

Experience working with literally homeless population; youth and youth‑headed households.

Crises intervention skills.

Position Type Full‑time, 40 hours per week (Monday through Friday). Some evenings and weekends may be required. Hourly/Nonexempt.

Working Conditions & Physical Requirements The case manager must be willing to use a vehicle for service provision throughout a seven‑county area and work in close proximity to staff at Bannock Youth & Family Center. Candidates must be able to perform essential job functions with or without reasonable accommodation.
