Pharmacist Specialist - Primary Care (Home Assessment Pathway) Health and Social Care Moray – Permanent Band 7 (0.6 WTE, 22.2 hours per week).

Responsibilities

Work alongside the Home Assessment Pathway Team to review patients' understanding and need for medication.

Carry out polypharmacy reviews to rationalise medication regimes.

Work across the Moray area and collaborate with the Pharmacy Management Team based in Elgin.

Utilise Independent Prescribing (IP) qualification where applicable.

Qualifications & Experience

Registered Pharmacist with experience in Primary Care.

Band 7 experience preferred.

Experience in polypharmacy reviews and medication rationalisation.

Independent Prescribing qualification desirable.
