Essential Duties and Responsibilities

Conduct assessments or screenings with clients to evaluate the nature of their presenting problems in order to determine their needs and develop appropriate treatment plan goals.

Develop and provide individual treatment plans; monitor, evaluate and document client’s progress.

May have responsibility for intake documentation. Prepare progress notes, event notifications and other documentation regarding clients in accordance with regulatory requirements and company policies and procedures.

Monitor case load daily to ensure required documentation is completed timely; including, but not limited to, assessments, treatment plans, case reviews and discharge plans.

Provide direct support services to include engaging clients in Independent Living Skills (ILS) activities, health and wellness promotion, personal assistance services, rehabilitation and vocational skills, training and development services, transportation services and crisis management.

Facilitate coordination of care to include case review, therapeutic interventions, and client progress in treatment.

Participate in clinical staff meetings, trainings and other activities as required.

Complete administrative tasks which may include work orders, copying and faxing.

Qualifications

A bachelor’s degree in the behavioral health field and one year of relevant experience in a behavioral health setting; OR

A non-behavioral health field bachelor’s degree and two year of relevant experience in a behavioral health setting.

Must have ability to perform First Aid or CPR as necessary and appropriate.

Knowledge in use of Electronic Health Records - NextGen a plus.

Experience working with diverse populations preferred.

Proficient in Microsoft Word, Excel, PowerPoint, Outlook.

Minimum typing speed of 25 wpm

Excellent oral and written communication skills.

This role is a non-driving position. This position is held at one location and does not require travel to various Terros Health centers. May be 18 years of age and with less than two years’ driving experience or no driving experience.

Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.

Must pass background check, TB test and other pre-employment screenings.
