Travel is required to provide care in patients’ private homes and residential facilities throughout Greater Boston and parts of MetroWest.
Responsibilities
Provide comprehensive psychosocial care to children and families with life-limiting illnesses.
Make home visits, facilitate referrals and admissions to services, and offer counseling support.
Qualifications
MSW required, LICSW preferred.
Pediatric or inpatient social work experience required.
Understanding of serious advancing/terminal illness.
Psychosocial assessment and treatment expertise.
Excellent organizational and time‑management skills.
Clear communication with patients, families, and clinicians.
Collaborative team contributor.
Must have a car and be willing to make independent home visits.
