Director, Value Based Care/Population Health for BJC HealthCare We are seeking a leader with deep experience in value based care/population health, ACOs, Medicare/CMS value programs, and value based contracting.

Role Summary: The Director will lead the execution of value based care strategies, drive operational excellence in quality results, clinical outcomes, and reduce total cost of care for BJC Medical Group.

Responsibilities

Manage onboarding, hiring, training, workload management, performance evaluations, and professional development plans for the team.

Develop and adhere to departmental staffing, revenue, and expense budgets. Respond to changes that may affect budget goals.

Perform new business activities to achieve assigned strategic objectives related to physician growth, increased utilization in service lines, strengthen community awareness, and develop employer activities.

Provide consultation and support to improvement teams and departments related to transformation and performance improvement initiatives.

Plan, develop, and direct operations of case management and social work and integrate services with other departments.

Monitor clinical outcomes and processes of care. Provide current data, information, and develop appropriate reports and documents in a timely manner.

Support and coordinate a performance improvement process to promote exceptional clinical outcomes while ensuring compliance with internal and external standards.

Minimum Requirements

Bachelor's Degree in Business/Healthcare Administration or related field.

10+ years of experience leading value based care or population health initiatives.

10+ years of supervisory experience managing a team.

Preferred Qualifications

Master's Degree in Business/Healthcare Administration or related field.
