A Brief Overview
Collaborates with all members of the health care team, patient, and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulate patient- and family-centered discharge plans. Serves as a resource to others in the work team and department.
What You Will Do
Provides psychosocial assessment and intervention to assist patients and families, including those with complex needs.
Provides age-specific care to patients and families across all age groups.
Develops, documents, and implements comprehensive plans based on identified psychosocial and discharge needs.
Develops plans with attention to individual patient and family goals.
Provides linkage to community resources and UH network services to facilitate optimal care.
Communicates and collaborates with the work team using a multidisciplinary approach.
Provides guidance and consultation to peers and team members regarding routine psychosocial issues and discharge planning.
Supervises social work interns if LISW eligible, completing supervision hours as required.
Additional Responsibilities
Performs other duties as assigned.
Complies with all policies and standards.
Refers to departmental documentation during orientation for specific duties and responsibilities.
Must abide by all requirements to safely and securely maintain Protected Health Information (PHI), including annual training, UH Code of Conduct, and policies and procedures regarding PHI use.
Education
Master's Degree from an accredited school of Social Work (Required)
Work Experience
Hospital or health agency experience (Preferred)
Licenses and Certifications
Licensed Social Worker (LSW) (Required)
Licensed Independent Social Worker (LISW) within 1 year (Preferred)
Position Details and Preferred Qualifications
The Social Worker performs evaluations to determine admission or discharge in the Psych Emergency Access Center, which serves as the evaluation and transfer center for psychiatric patients across all entity emergency departments. Patients are seen in-person and via telemedicine. The environment is fast-paced, with patients seen within one hour of referral. The ideal candidate will be autonomous, reliable, a critical thinker, experienced in crisis management, passionate about psychiatric care, and a self-starter. The position is for 40 hours/week, night shifts, Monday-Friday, with rotating weekends and holidays, all via telehealth.
