Overview
Social Worker 2 - PRN: Peds & OB (250006JE) Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource to others in the work-team and department. What You Will Do
Provides psychosocial assessment / intervention to assist patients/families (including clinically complex). Provides age specific care to patients/families in all age groups. Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs. Develops plans with attention to individual patient and family goals. Provides linkage to community resources and UH network services to facilitate optimal care. Communicates/collaborates with the work team using a multidisciplinary approach. Provides guidance/ consultation to peers/team regarding routine psychosocial issues and /or discharge planning issues. Provides supervision to social work interns if LISW eligible (completion of supervision hours). Additional Responsibilities
Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. Education
Master\'s Degree from an accredited school of Social Work (Required) Work Experience
Hospital/Health Agency experience. (Preferred) Licenses and Certifications
Licensed Social Worker (LSW) (Required) Licensed Independent Social Worker (LISW) (Preferred within 1 Year) Preferred Qualifications/Position Details
At least 1 year of prior work with pediatrics and/or OB is strongly preferred. This PRN position is needed to cover pediatrics and OB at any location (i.e. main campus, the outpatient Rainbow Center for Women & Children (Midtown), and any of our community hospitals). The role of the social worker is critical to provide psychosocial assessments, crisis interventions, and resource referrals, as well as timely, efficient, safe discharge planning, and supportive counseling services in order to meet the needs of patients and families. Examples of issues may include domestic violence, child abuse, poverty, depression, homelessness, behavior problems, teen pregnancy, etc.
Shifts are 8:30am-5:00pm; weekdays are the greatest need but there could be occasional opportunities for a Sunday shift.
