The Social Worker 2 role within the University Hospitals Geauga Medical Center offers a unique chance to become part of a dynamic, collaborative interdisciplinary team. This position adds value to patient care and significantly assists OB patients with various issues.

A Brief Overview
Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource to others in the work-team and department. What You Will Do
Provide psychosocial assessment / intervention to assist patients/families (including clinically complex) and provide age-specific care to patients/families in all age groups. Develop, document and implement a comprehensive plan based on identified psychosocial needs and/or discharge needs. Develop plans with attention to individual patient and family goals. Provide linkage to community resources and UH network services to facilitate optimal care. Communicate/collaborate with the work team using a multidisciplinary approach. Provide guidance/consultation to peers/team regarding routine psychosocial issues and/or discharge planning issues. Provide supervision to social work interns if LISW eligible (completion of supervision hours). Additional Responsibilities
Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. Education
Master's Degree from an accredited school of Social Work (Required) Work Experience
Hospital/Health Agency experience. (Preferred) Licenses and Certifications
Licensed Social Worker (LSW) (Required) Licensed Independent Social Worker (LISW) within 1 Year (Preferred) Position Details/Preferred Qualifications
At least one year of experience is strongly preferred. Previous work with OB patients is also highly preferred. This position provides social work services for Macdonald Women’s hospital beds located at our Geauga Medical Center in Chardon. The role performs psychosocial assessments, supportive counseling, crisis intervention, and provides resource referrals and resources. This role is essential as it impacts timely and safe discharges, as well as patient and provider satisfaction.
The schedule is Monday-Friday, 8:00am-12:00pm.
