Location: Lexington, KY
Schedule : Monday-Friday 8am-5pm

About the role:
A Social Worker in this position is a member of the community-based palliative care (CBPC) team and provides direct casework and counseling services to the patient and family unit wherever they may be in the palliative care service. The Palliative Care Social Worker supports both our adult and pediatric palliative care teams and will provide an assessment of psychosocial problems, define treatment goals, and work to alleviate such problems. The Palliative Care Social Worker serves as part of the interdisciplinary team and shares responsibility for other aspects of patient care except where limited by licensure. This position will also plan and coordinate patient care with the palliative team under the direction of team leaders, program leaders, and the Chief Medical Officer.
Requirements
Is a graduate of a Bachelor’s or Master’s program in Social Work accredited by the Council of Social Work Education; and is licensed by the state of Kentucky as a Licensed Social Worker (LSW), a Certified Social Worker (CSW), or a Licensed Clinical Social Worker (LCSW).
A minimum of two years’ social service experience in a health care setting or social service agency. Experience working on an interdisciplinary team preferred.
Knowledge of the process of death, dying, and bereavement.
Knowledge and basic understanding of the psychosocial and legal operatives in terminally ill patients and their families.
Knowledge and basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary healthcare team.
Ability to exercise judgment and discretion in confidential matters.
Must be able to make home visits to patients and families in a variety of settings which may include mobile homes, apartments, homes, nursing facilities, and condominiums that may not have elevators.
Must be able to drive for extended periods of time with no ill effects.
Must be flexible, have good written and oral communication skills. Must have the ability to give and receive constructive feedback.
Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.
Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
