Overview Social Worker LCSW (No Weekends; No Call) - Population Health - Kings Hwy at Ochsner LSU Health, located in Shreveport, LA. The role is responsible for bio-psychosocial assessments, patient education, evaluation, referral to appropriate community resources, and supportive and crisis interventions. It coordinates and implements complex discharge plans; collaborates with the multidisciplinary team to reassess and respond to patient and caregiver needs, including emotional, social, cultural and spiritual aspects in relation to the patient’s medical condition. The position may practice clinical social work within the agency under the supervision of an LCSW employed by the company.

Responsibilities

Interviews and assesses patients and assists throughout patient care plan.

Identifies and provides helpful resources to patients and families.

Maintains patient records and documents and communicates critical information to the medical team.

Participates in on-call rotation.

Adapts behavior to the specific patient population, including privacy, introducing services, obtaining permissions, and adapting explanations and communication style as needed.

Performs other related duties as required.

Assumes responsibility for bio-psychosocial assessments, patient education, evaluation, referral to community resources and supportive/crisis interventions; coordinates and implements complex discharge plans and collaborates with the multidisciplinary team to respond to patient and caregiver needs across emotional, social, cultural and spiritual dimensions.

Qualifications

Education: Required - Master’s degree in Social Work (MSW) from a CSWE-accredited program.

Work Experience: Required - 5 years of related experience. Preferred - Two years of experience in a medical social work field.

Certifications: Required - Licensed Clinical Social Worker (LCSW) as verified by the Louisiana State Board of Social Work Examiners (LABSWE) and able to engage in independent practice. OR CSW or RSW certificate as verified by LABSWE. Preferred - LMSW as verified by LABSWE or scheduled to pass the LMSW exam within 6 months of employment.

Knowledge, Skills and Abilities (KSAs)

Good knowledge of common disease entities and medical terminology to understand acute and ongoing needs of patients and families.

Excellent psychotherapeutic skills.

Superior interpersonal skills to interact with patients, families, multidisciplinary team members and community agency personnel.

Advanced knowledge of an acute and chronic care medical setting, medical terminology, and the psychosocial and economic effects of illness and/or trauma on the patient and their support system.

Advanced knowledge of managed care and public programs (Medicare, Medicaid, Medicare Advantage, and commercial insurance plans).

Proficiency in using computers, software and web-based applications.

Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.

Ability to travel between facilities and work a flexible schedule (e.g., 24/7, weekends, holidays, on-call).

Physical and Environmental Demands

Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.

Normal routine involves no exposure to blood, body fluid or tissue, but exposure or potential for exposure may occur.

The incumbent works with patients who have known or suspected communicable diseases and may enter isolation rooms. There may be an occupational risk for exposure to communicable diseases.

Because the incumbent works within a healthcare setting, there may be occupational risk for exposure to hazardous medications or hazardous waste within the environment.
