Job Overview
This position is responsible for bio‑psychosocial assessments, patient education, evaluation, referral to community resources, and supportive and crisis interventions. The role applies thorough knowledge of Ochsner and community‑based services to coordinate and implement complex discharge plans, collaborating with the multidisciplinary team to evaluate and respond to patient and caregiver needs—including emotional, social, cultural, and spiritual aspects related to medical conditions.
Within the agency, practice of clinical social work may be performed under the supervision of an LCSW employed by Ochsner Health System.
Responsibilities
Conduct patient assessment interviews and coordinate discharge plans.
Provide referrals and identify helpful resources for patients and families.
Maintain patient records, document all care, and communicate critical information to the medical team.
Maintain professional and technical knowledge and required certifications through continuing education and development.
Participate in on‑call rotation.
Adapt communication style to specific patient populations, respecting privacy, tailored introductions, and appropriate use of permissions.
Other related duties as required.
Job Duties

Maintain patient records and documents and communicate critical information to the medical team.
Maintain professional and technical knowledge and required certifications through continuing education and development.
Utilize management resources.
Participate in on‑call rotation.
Adapt behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
Qualifications
Education : Master's degree in Social Work (MSW) from a university accredited by the Council on Social Work Education (CSWE).
Work Experience : None required.
Preferred : Two years experience in a medical setting.
Certifications (Required) : Certified Social Worker (CSW) as verified by LABSWE or Registered Social Worker (RSW) certificate; must pass licensure exam within six months of employment.
Certifications (Preferred) : Licensed Master Social Worker (LMSW) as verified by LABSWE; must schedule and pass exam within six months of employment.
Knowledge, Skills, and Abilities (KSAs)
Computer skills and dexterity for data entry and retrieval.
Proficiency with Windows‑style applications, keyboard, and various software packages specific to the role.
Effective verbal and written communication skills; ability to present information clearly and professionally to a variety of individuals.
Working knowledge of common disease entities, disorders, and medical terminology to understand patient needs.
Understanding of an acute and chronic care medical setting, psychosocial, and economic effects of illness and trauma.
Knowledge of healthcare systems and public programs such as Medicare, Medicaid, Medicare Advantage, and commercial insurance plans.
Strong interpersonal skills to interact effectively with patients, families, multidisciplinary team members, departmental peers, and community agency personnel.
Physical and Environmental Demands Medium work: occasional lifting of 20‑50 pounds, frequent lifting of 10‑25 pounds, and occasional lifting up to 10 pounds constantly. Physical demands exceed those for sedentary work. May require walking, standing, and sitting for prolonged periods. Must be able to travel between facilities and work a flexible schedule (24/7, weekend, holiday, on‑call). Exposure to communicable diseases may occur; protective equipment and protocols will be provided. Mechanical lifting devices and team lifts should be utilized when possible.
