General Purpose Under general supervision, provide outreach and case management services to specific populations within the El Paso County area.
Typical Duties Plan, implement and assess a preventative health client‑centered program. Responsibilities include implementing an individual service plan for persons living with or at risk for infectious diseases, providing direct support through one‑to‑one contacts, conducting case management activities such as follow‑up, service plan review, and re‑evaluations, performing comprehensive assessments, client advocacy, and goal setting. Conduct home visits as necessary, teach and model problem solving and assertive communication, provide preventive health screenings and counseling, perform venipuncture and finger sticks, and collaborate with other Public Health programs. Educate individuals and groups on the epidemiology of infectious diseases, modes of transmission and prevention, maintain confidentiality, stay current on vector‑borne diseases and reproductive health, and provide referrals. Administer satisfaction surveys, develop educational materials, conduct community education, document records, and compile data for reports.
Education and Experience A Bachelor's degree or higher in Social Work, Behavioral Sciences, Health Education, Community Health Education, Public Health Education, School Health Education, or a related field and two (2) years of experience in the healthcare field, providing case management, health education in a public health, clinic and/or school setting, or direct contact with the public, patient or provider in a communicable disease control, epidemiology, or preventive health program.
Licenses and Certificates Texas Class "C" Driver's License or equivalent from another state.
