Overview
Social Worker MSW - Case Management at Ochsner Baptist, Full Time with a $15k sign-on bonus available. Conduct patient assessment interviews, provide referrals, and coordinate discharge plans. Identify and provide resources to patients and families. Maintain patient records and document and communicate critical information to the medical team. Maintain professional knowledge and required certifications through continuing education and development. Utilize management resources and participate in on-call rotation. Adapt behavior to the patient population, ensuring privacy, appropriate introductions, and clear communication about services and procedures. Qualifications
Education:
Required – Master\'s degree in Social Work (MSW) from a CSWE-accredited program. Work Experience:
Required – None. Preferred: Two years of experience in a medical setting. Certifications:
Required – Certified Social Worker (CSW) or Registered Social Worker (RSW); must take and pass licensure exam within 6 months of employment. Preferred – Licensed Master Social Worker (LMSW) with LABSWE. Knowledge, Skills & Abilities (KSAs):
Proficiency with data entry and Windows-based software; strong verbal and written communication; knowledge of disease entities, medical terminology, and psychosocial/economic effects of illness; understanding of healthcare systems and public programs; strong interpersonal skills for working with patients, families, and the multidisciplinary team. Job Duties
