Social Worker MSW – Case Management – Ochsner Baptist – PRN
Responsible for bio‑psychosocial assessments, patient education, evaluation, referral to appropriate community resources, and crisis interventions. Applies comprehensive knowledge of company and community‑based services to coordinate and implement complex discharge plans; collaborates with the multidisciplinary team to (re)evaluate and respond to patient and caregiver needs, including emotional, social, cultural, and spiritual aspects related to the patient’s medical condition. May practice clinical social work within the agency under supervision of an LCSW employed by the company.
Education Master’s degree in Social Work (MSW) from a university accredited by the Council on Social Work Education (CSWE).
Work Experience None required.
Preferred Experience Two years experience in a medical setting.
Certifications Required: Certified Social Worker (CSW) as verified by LABSWE or Registered Social Worker (RSW) certificate. Must take and pass licensure exam within 6 months of employment.
Preferred: Licensed Master Social Worker (LMSW) as verified by LABSWE or schedule and pass exam within 6 months of employment.
Knowledge, Skills, and Abilities (KSAs) Computer skills for data entry and retrieval.
Proficiency with Windows applications, keyboard, and various software packages.
Effective verbal and written communication skills.
Working knowledge of common disease entities, disorders, and medical terminology.
Working knowledge of acute and chronic care medical settings and the psychosocial/economic effects of illness.
Knowledge of health care systems and public programs (Medicare, Medicaid, etc.).
Strong interpersonal skills for dealing with patients, families, and team members.
Job Duties Conduct patient assessment interviews, provide referrals, and coordinate discharge plans.
Identify and provide helpful resources to patients and families.
Maintain patient records and document critical information to the medical team.
Maintain professional and technical knowledge and required certifications through continuing education.
Utilize management resources.
Participate in on‑call rotation.
Adapt behavior to the patient population, respecting privacy and communication style.
Compliance & Privacy Maintains and reports any known unethical or questionable behaviors. Ensures compliance with all laws, regulations, and standards pertaining to patient privacy and safety.
