Position Summary Responsible for assessing, planning, coordinating and evaluating the care of patients.

Scheduled Hours 40 hours per week.

Primary Duties & Responsibilities

Conduct a comprehensive sensitive assessment with the client and family/support network to develop a plan of care to meet identified needs.

Arrange and coordinate service referrals for the client/family as needed; provide ongoing monitoring of the activities implemented to carry out the plan of care. Refer clients/families to public benefits programs as eligible.

Provide regular contact with enrolled clients and document all contacts and service coordination activities in client records as required by program guidelines.

Educate clients/families about the basic medical and psychosocial aspects of disease in order to optimize adherence to care.

Provide proactive follow up, tracking symptoms and treatment responses.

Adhere to all applicable state laws and the department’s rules and regulations regarding programs.

Stay abreast of new resources, programs and developments; participate in continuous training and education, including attendance at staff meeting and community meetings as assigned.

Education Master’s degree or a combination of education and/or experience may substitute for minimum education.

Certifications

Basic Life Support: American Heart Association, Basic Life Support – American Red Cross.

Work Experience No specific work experience is required for this position.

More About This Job

Basic Life Support certification must be obtained within one month of hire date.

Basic Life Support (online BLS certifications without a skills assessment component) are not sufficient to meet the BLS requirements.

Preferred Qualifications No additional education or certification unless stated elsewhere in the job posting.

Grade C10

Salary Range $52,600.00 – $78,900.00 annually
