SUMMARY: Reporting to the Social Work Manager, the Social Worker LSW evaluates, diagnoses and treats a wide range of psychosocial problems, applying individual clinical social work skills to assist patients and their families in resolving personal and environmental problems attributable to mental or physical illness, which prevent them from achieving their highest level of psychosocial functioning. The Social Worker assigned to inpatient units participates collaboratively with unit leadership to support patient flow. Adheres to social work professional standards of practice based on NASW Code of Ethics. Responsibilities
Consistently apply corporate values of respect, honesty and fairness and pursue excellence in improving the health status of the region through the provision of accessible, high-value services within a comprehensive academic health system.
Know and act in accordance with the Brown University Health Corporate Compliance Program and Code of Conduct.
Complete bio-psychosocial evaluations to remove barriers to treatment, facilitate discharge, and improve resource utilization.
Provide consultation regarding patient background, psychosocial functioning and needs to medical and support staff to plan a comprehensive care program, enhance treatment compliance, and coordinate care for a positive patient experience.
Develop treatment plans, initiate clinical interventions using individual and group modalities, and analyze progress to maximize health outcomes.
Document social work activity in patients’ medical records per departmental standards.
Participate in conferences and collaborative care rounds to improve care coordination and safe transitions to communities and less restrictive settings.
Educate patients and families to understand and follow medical and social work recommendations to restore functioning.
Assess advocacy needs and coordinate appropriate advocacy activities within the hospital and community, focusing on self-determination and enhanced social supports.
Provide field placement supervision and evaluation for BSW and/or MSW candidates (LICSW required or at least 5 years of relevant experience).
Complete case-related statistics and reports per departmental standards.
Engage in hospital-wide, departmental, and community studies to educate on psychosocial issues and reduce barriers to service delivery.
Remain current on community resources and access them as needed; maintain professional expertise through ongoing education.
Participate in quality assurance activities in line with department, hospital, and Joint Commission standards.
Adhere to Brown University Health, hospital and department policies to maintain a safe care environment and comply with laws, regulations and standards.
Supervisory responsibility: provide functional guidance to Social Work Interns with recommendations to pursue LICSW.
Qualifications
Minimum Qualifications
Master’s Degree in Social Work from an accredited graduate school and current RI licensure (License from State of RI, Board of Social Work Licensure).
Licensure: Licensed Clinical Social Work (LCSW); eligible for Licensed Independent Clinical Social Work (LICSW) after two years or 2000 hours of post-master supervised practice.
Demonstrated knowledge to provide care considering aging, development, and cultural patterns.
Experience : Between six months and one year of clinical experience in health or mental health settings with relevant populations; ability to interact with patients, families, medical staff and outside agencies.
Work Environment Works in a normal office environment with exposure to patient care.
Pay Range $28.95-$47.76
