Standard Hours:
40 per week
Shift:
Monday–Friday 08:00–16:30, 9 weekends, 1 holiday per year
Sign on bonus:
$5,000
Responsible for coordinating the discharge plan of care for all patients during their hospital admission. Coordinates the discharge plan from admission through discharge follow‑up, provides biopsychosocial assessments and updates, and collaborates with multidisciplinary teams in Care Coordination Rounds.
Responsibilities
Coordinate discharge planning from admission to post‑discharge follow‑up.
Provide biopsychosocial assessments and develop discharge care plans.
Update and communicate changes to the patient’s care team.
Collaborate with multidisciplinary teams during Care Coordination Rounds seven days a week.
Experience A minimum of two years experience in an acute‑care medical social work setting is preferred. Casework experience in medical social work is an advantage.
Education Master’s degree in Social Work required.
Skills
Interpersonal skills for information gathering, counseling, and effective patient/family interaction.
Analytical skills for assessing patient needs and developing discharge planning solutions.
License Current State of Wisconsin license as a social worker required.
