Social Worker (MSW)
Join Immanuel Pathways PACE® as a Social Worker (MSW) to support participants and families at 7700 Hickman Road, Windsor Heights, IA 50324. This full‑time position runs Monday through Friday 8:00 am to 5:00 pm, with no weekend or holiday work.
Schedule & Compensation Pay will be based upon experience, certifications, skills, and education.
Requirements
Legally authorized to practice social work in Iowa (currently licensed, certified, or registered).
Practice within the scope of authority allowed in Iowa.
Abide by the philosophy, practices, and protocols of the PACE organization.
Meet job‑specific competencies for the Social Worker prior to assuming participant care.
May cover a counterpart at a different PACE Pathways site (only in Iowa) to assess potential or current participants.

Job Duties & Skills Required Plans, organizes, and implements social services to Immanuel Pathways participants and families. Responsibilities include assessment, treatment, teaching, and counseling to participants, caregivers, or other appropriate representatives. Social work interventions may include individual participant contacts; appropriate collateral contacts; participant and family education; assessment and counseling; provision of resources; ongoing case management; advocacy to ensure participant and caregiver needs are met and addressed; and disenrollment procedures.
Qualifications
Master’s Degree from an accredited school of social work.
Two (2) years of experience working on a multidisciplinary team in a hospital, nursing home, or community‑based setting.
Currently authorized to work in the United States on a full‑time basis.
