#DegreesNYC . Organization: Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) serve approximately 30,000 New Yorkers in Manhattan. We are leaders in human services supporting children, youth and families, homeless individuals, and older adults, with programs focused on fair access to opportunities and improved quality of life. Program Description: Lincoln Square Neighborhood Center (LSNC) is a multigenerational program dedicated to educational, recreational, and cultural enrichment for the Lincoln Square community, with a primary focus on Amsterdam Houses and Amsterdam Addition. LSNC Aging Services provides comprehensive support to over 800 active members, including nutritious meals, wellness programs, educational and arts programming, and social services. NORC services at LSNC serve residents 60+ in the Amsterdam Houses/Addition to promote aging in place via case management, benefits assistance, advocacy, health promotion, and social engagement. Position Summary: The Senior Center Case Manager supports older adults in maintaining independence and aging in place through individualized case assistance, access to resources, and advocacy to help seniors thrive safely at home. Schedule
Monday–Friday, 35 hours per week (8:00 am–4:00 pm) with occasional late hours (4:00–6:00 pm) and once-a-month weekend coverage for Cooling Center Administration (summer rotation). Salary Range
$48,000–$52,000 per year. Note:
Salary offers are based on years of relevant experience and qualifications. Responsibilities
Provide comprehensive support and advocacy for older adults on issues including entitlements, healthcare, mental health, food assistance, housing, financial security, and social services. Conduct annual assessments and reassessments, as well as home visits for homebound seniors, to evaluate needs and connect them with services. Assess clients for cognitive changes and implement appropriate interventions, referrals, and supports to ensure safety and quality of life. Identify and respond to elder abuse, neglect, or financial exploitation, coordinating with Adult Protective Services, law enforcement, and legal services when needed. Occasionally accompany seniors to medical, social service, or legal appointments, including hospital visits when admitted. Maintain accurate documentation of services and client interactions in NYC Aging online database in line with program, agency, and funder guidelines. Plan and facilitate support groups, educational workshops, and wellness sessions. Connect caregivers to support services, including individual consultations, counseling, and referrals to caregiver programs. Educate family members on aging-related challenges and community resources to support aging in place. Assist families with long-term care planning, Medicaid applications, and elder care navigation. Assist seniors in scheduling and coordinating medical appointments, ensuring transportation and follow-up reminders. Monitor health and well-being, respond to crises, and coordinate emergency services or hospital care when needed. Collaborate with healthcare providers and community resources to ensure continuous, comprehensive care. Oversee and coordinate friendly visiting and telephone reassurance services to ensure regular social contact for isolated seniors. Encourage participation in center activities, social programs, and community initiatives. Participate in staff meetings, case conferences, and professional development; contribute to program evaluation and service improvements. Assist in daily operations of LSNC Aging Services and support client engagement programs; conduct outreach to expand participation. Perform other duties as assigned by the supervisor consistent with primary responsibilities. Qualifications & Educational Requirements
Bachelor’s degree (BA, BS, or BSW) in social work, psychology, gerontology, or related field; MSW preferred. Minimum of two years' professional experience with older adults in case management, social services, or healthcare settings. Fluency in a second language (Spanish, Cantonese/Mandarin, or Russian) preferred. Strong sensitivity to the needs and strengths of older adults; ability to manage multiple priorities in a dynamic environment. Excellent written and verbal communication skills with clear case notes. Strong problem-solving abilities and the capacity to respond effectively to diverse situations. Experience engaging with older adults who may have disabilities or mental health conditions. Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and quick learning of electronic databases for case management. Prior experience with DFTA Peer Place (STARS) database preferred. Physical & Work Environment
Ability to circulate within the senior center, engage with clients in common areas, and accompany to appointments as needed. Ability to stand and walk frequently during programming activities. Ability to travel locally for home visits and hospital visits. Work environment includes a senior center located in a NYCHA facility, with shared office space on the second floor (accessible by elevator).
