Admissions, Discharge, and Transfers of residents Medication requisitions with pharmacy and physicians Medication administration Following infection control, emergencies, and other center policies Collaborating with residents' interdisciplinary team to provide the best outcomes Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy Working under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment Delivering nursing care to patients/residents requiring long-term or rehabilitative care Collecting patient/resident data, making observations, and reporting pertinent information related to the care of the patient/resident Implementing the patient/resident plan of care and evaluating the patient/resident response, according to state-specific regulations Directing and supervising caregiving by other nursing personnel in selected situations, in accordance with state-specific regulations Maintaining knowledge of necessary documentation requirements Maintaining knowledge of equipment setup, maintenance, and use, e.g., monitors, infusion devices, drain devices, etc. Maintaining confidentiality and patient/resident rights regarding all patient/resident/personnel information Licensed Practical Nurse (LPN) QUALIFICATIONS:

LPN (Licensed Practical Nurse) licensed by the State CPR Certification required Two Years prior nursing experience preferred
