Bowery Residents' Committee . Continue with Google Duties/Responsibilities: Coordinate healthcare and medical case management for clients. Perform basic health assessments, check vitals, blood pressure, and weight. Provide ongoing health monitoring and support. Make referrals and follow-up for medical, substance use, and psychiatric treatments. Observe, monitor, and document client medication management. Process physicians' orders and obtain prescriptions. Collaborate with medical and psychiatric providers and hospitals. Develop and facilitate health-related educational groups and individual education. Participate in team meetings and case conferences. Respond to medical emergencies, provide first aid/CPR, and Narcan overdose reversal. Address other crises as needed and ensure regulatory compliance. Hours:
Full-time, 37.5 hours/week, including Saturday 8am-6pm and Wednesday-Friday 12pm-10:30pm. Qualifications: NYS LPN license required. Bachelor's Degree preferred. Experience with homelessness, mental illness, or substance use disorder is a plus. Proficiency in MS Office. Bilingual skills are a plus. CPR certification or willingness to obtain it.
