Responsibilities:
Responsible for direct patient care for assigned patients.
Evaluates the clinical needs of the patient; develops, implements, and revises the patient's plan of care; documents accordingly.
Utilizes clinical grouping guidelines for interdisciplinary utilization and initial visit frequency data entry.
Ensures OASIS assessments accurately reflect documentation.
Updates Clinical Manager on referrals to community resources.
Participates in interdisciplinary patient case conferences and evaluates the effectiveness of the care plan.
Supervises and documents Home Health Aide care every 14 days.
Notifies MD and care team of significant patient condition changes.
Completes admission and other paperwork promptly.
Provides clear, concise narrative documentation of skilled care, teaching, and patient/caregiver responses.
Obtains and clarifies MD orders for all care delivered.
Coordinates discharges with the interdisciplinary team, informing patient/MD and completing discharge paperwork.
Performs other duties as assigned.
Passes VNA pre-hire medication test.
Holds current RN license in Massachusetts and related credentials.
Minimum one year of nursing experience or 6 months with a BSN.
CPR certification required.
Preferred: Home care and IV experience, computer skills, BSN.
Position Purpose: Performs comprehensive nursing assessments upon patient admission, considering physiological, psychological, environmental factors, and health history to ensure safe, quality care. Collaborates with the clinical team to integrate assessment data and identify patient problems for care planning.
Schedule Details: 37.5 hours/week, rotating days, 8 am-4 pm, rotating weekends and holidays. Area: Upper Cape.
