Overview

As a registered nurse in the Clinical Nurse Liaison role, you will provide quality care to patients from diverse socioeconomic and cultural backgrounds. You’ll develop relationships with referral sources such as providers, hospitals, long-term care facilities, or doctor offices to enhance the admission process and provide community support. You’ll need to be familiar with marketing and strategic goals for the company and be confident and knowledgeable about the services offered by Lower Cape Fear LifeCare.

Able to provide service recovery and excellent customer service skills to ensure satisfaction.

Able to manage multiple tasks simultaneously and quickly pivot from one task to another as needed.

Understand hospice eligibility and advise on the most appropriate care settings.

Public speaking is vital, as you’ll conduct educational in-services with medical staff or community groups on topics such as Hospice 101 – Eligibility and Philosophy, End-of-Life and Symptom Management, and Specific Disease States (e.g., dementia, heart failure, COPD, end-stage renal disease).

Spend time one-on-one with patients and their families to act as a consultant and provide education on identifying signs and symptoms, common medications used in hospice care, and what to expect at end-of-life (while not providing direct patient care).

Collaborate and coordinate with external medical staff and referral sources to determine patient eligibility for hospice, palliative care, or Memory Partners program.

Coordinate transition of care for patients who meet admissions criteria into inpatient hospice care centers, in-hospital hospice, outpatient case managers, to receive hospice care at home, or hospice services in a facility setting.

Coordinate transition of care activities including obtaining consent to treat, discharge planning with physicians and case managers, and coordinating with outside vendors (e.g., transport) to organize patient pickup and arrival times.

Complete documentation for new referrals and follow-ups after each visit and start the admissions process with LCFL staff, including ordering DME and other supplies.

What we’re looking for

Education:

Graduate of an accredited school of nursing (AD, Diploma, or BSN). Degree in business, marketing, or related field preferred.

Licensure / Certifications:

Current license to practice professional nursing in the State of North Carolina or South Carolina, depending on position location.

Experience:

Two years of experience; hospice or home care preferred. Experience in marketing, public affairs, community relations, or hospital liaison nursing is preferred.

Essential Technical / Motor Skills:

Clear speech to communicate with patients, families, physicians, and staff. Good command of English. Computer experience helpful.

Interpersonal Skills:

Ability to develop positive interactions with patients, families, physicians, staff, and other health professionals to educate about hospice services and end-of-life care. Strong verbal and written communication skills. Highly self-motivated and flexible.

Essential Physical Requirements:

Ability to move freely; ability to work long hours and manage stressful situations.

Essential Mental Abilities:

Ability to define problems, collect data, establish facts, and draw valid conclusions; capacity for both short- and long-term memory.

Essential Sensory Requirements:

Touch, sight, hearing; ability to organize thoughts, facts, and ideas; ability to translate written word.

Exposure to Hazards:

May be exposed to blood and body fluids, infectious diseases, needle puncture wounds; may encounter situations posing personal safety risks.

Hours of Work:

Flexibility with schedule required; weekends, holidays, evenings, and early mornings may be required.

Population Served:

Physicians, RNs, facility administrators, discharge planners, social workers, other health professionals, insurers, public and community members; all patient populations from infants to elderly with life-limiting illness.

Driving:

Must be able to drive and have reliable transportation; may be subject to the Safe Driver Policy.
