Location: Wilmington, NC. Salary: $68,000 - $90,000.
Responsibilities
Provide quality patient care as a registered nurse to patients from diverse socioeconomic and cultural backgrounds.
Develop relationships with referral sources—specific providers, hospitals, long‑term care facilities, or doctor's offices—to enhance the admission process and provide community support.
Be familiar with the company’s marketing and strategic goals and be confident in explaining the services offered by Lower Cape Fear LifeCare.
Provide excellent customer service and service recovery to ensure patient and family satisfaction.
Manage multiple tasks simultaneously and pivot quickly from one task to another as needed.
Understand hospice eligibility and advise on the most appropriate care setting.
Conduct educational in‑services and presentations for medical staff or community groups on topics such as
Hospice 101 – Eligibility and Philosophy ,
End‑of‑Life and Symptom Management , and
Specific Disease States (e.g., dementia, heart failure, COPD, end‑stage renal disease) .
Educate patients and families—identify signs and symptoms, explain common medications used in hospice care, and discuss end‑of‑life expectations.
Collaborate with external medical staff and referral sources to determine patient eligibility for hospice, palliative care, or the Memory Partners program.
If a patient meets admissions criteria, coordinate the transition of care to one of our inpatient hospice centers, in‑hospital hospice, outpatient case management, or home hospice services.
Obtain consent to treat from the patient or primary caregiver; work with physicians and case managers on discharge planning; coordinate transportation; complete documentation for new referrals and follow‑up after each visit; and start the admissions process with LCFL staff, including ordering DME and other supplies.
Qualifications
Education: Graduate of an accredited nursing program (AD, Diploma, or BSN). A degree in business, marketing, or a related field is preferred.
Licensure: Current state license to practice professional nursing in North Carolina or South Carolina.
Experience: Minimum of two years’ experience, preferably in hospice or home‑care settings; experience in marketing, public affairs, or community relations is an advantage.
Technical skills: Ability to communicate clearly with patients, families, physicians, and staff; competency in computer use.
Interpersonal skills: Strong verbal and written communication, self‑motivation, and flexibility; ability to build positive interactions with healthcare professionals.
Physical & mental: Ability to move freely, work long hours, manage stressful situations; clear mental abilities for problem definition and data analysis.
Safety: Awareness of potential hazards, including blood and bodily fluids, infections, and needle punctures.
Schedule: Flexibility required; weekends, holidays, evenings, and early mornings may be needed.
Transportation: Reliable vehicle for driving duties; compliance with the Safe Driver Policy.
