About Upward Bound House (UBH)
Upward Bound House is dedicated to ending family homelessness in Los Angeles. With over 30 years of experience, we provide a continuum of housing and support services that help families and young parents transition from crisis to stability. As we launch our new DMH-funded interim housing program, we’re seeking passionate, experienced professionals to join our growing team.
Position Summary The LVN supports families—both Transitional Age Youth (TAY) and general—in a 24-hour interim housing program. Under the supervision of a Registered Nurse or physician, this role focuses on medication management, health monitoring, and medical coordination to ensure quality care in a trauma-informed setting.
Work Environment You’ll work closely with families who may be in crisis, experiencing substance use, or other challenges. This role requires empathy, the ability to remain calm under pressure, and the capacity to respond effectively in emergencies.
Key Responsibilities Medication Management
Administer prescribed medications and monitor effects
Maintain accurate Medication Administration Records (MARs)
Support medication refills, renewals, and reconciliation
Educate families on safe medication use
Health Monitoring & Care Coordination
Conduct health screenings and monitor vitals
Track physical and mental health status changes
Coordinate medical appointments and referrals
Serve as liaison between families and providers
Emergency & Crisis Response
Provide first aid and respond to medical crises
Collaborate with emergency services and internal teams
Maintain emergency medical supplies
Health Education & Support
Deliver health and wellness education to families
Support chronic condition management and preventive care
Provide age-appropriate resources to TAY families
Documentation & Compliance
Maintain nursing documentation and DMH reporting
Ensure HIPAA compliance and quality assurance
Participate in audits and ongoing training
Required Qualifications
Graduate of an accredited LVN program
2+ years of nursing experience (mental health/psychiatric meds preferred)
Active California LVN license
CPR/First Aid certified; current TB test
Valid driver’s license, insurance, and reliable transportation
Strong communication, crisis response, and documentation skills
Preferred Qualifications
Experience in residential or transitional housing
Familiarity with DMH and TAY populations
Bilingual (preferred based on community needs)
Proficient in EHR systems
