Description LGTC Group is looking for a bilingual (Spanish) case manager/social worker fluent in both English and Spanish for our Campbell location. Job Summary:

We are seeking a compassionate and dedicated Bilingual Case Manager or Social Worker fluent in Spanish and English to support clients from diverse backgrounds. This role involves assessing client needs, developing service plans, connecting individuals to community resources, and providing culturally competent support to ensure safety, stability, and well-being. Key Responsibilities:

Conduct intake assessments to determine client needs and eligibility for services.

Provide case management, including service planning, referrals, advocacy, and follow-up.

Maintain a manageable caseload while ensuring timely, client-centered support.

Coordinate with external agencies, healthcare providers, and community organizations.

Offer crisis intervention and support for clients experiencing housing instability, domestic violence, behavioral health issues, or other challenges.

Maintain accurate documentation, case notes, and reports in compliance with program and funding requirements.

Participate in multidisciplinary team meetings and case conferences.

Educate clients on available services and help them navigate complex systems (healthcare, legal, housing, etc.).

Deliver services in both English and Spanish, ensuring language access and cultural sensitivity.

Attend trainings, workshops, and supervision to stay informed on best practices and policies.

Requirements Requirements:

Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (Master’s preferred for clinical roles).

Bilingual (Spanish-English) fluency – verbal and written. 1–3 years of experience in case management, social work, or a related field. Strong cultural competency and experience working with Latino/Hispanic communities. Knowledge of community resources and public benefit systems. Excellent organizational, documentation, and communication skills. Ability to manage multiple priorities and work effectively in a fast-paced environment. Valid driver’s license and reliable transportation (if required for field work). Computer literacy, including familiarity with case management software and MS Office. Preferred Qualifications:

Master’s degree in Social Work (MSW) or related field. LCSW or other licensure (if providing clinical services). Experience working with immigrant or refugee populations. Trauma-informed care training or certification. Familiarity with HIPAA, mandated reporting laws, and client confidentiality standards.
