Overview
Case Manager, Criminal Justice Liaison at Family & Children’s Services. This role supports criminal justice involved clients and treatment within the agency, helping clients navigate the court system and access Mental Health and/or Substance Abuse Treatment. Acts as an advocate and liaison between the agency, JusticeLink, and the court system under the supervision of the Program Supervisor. Works with an interdisciplinary team including judges, court personnel, law enforcement, probation officers and other treatment providers. Responsible for communication flow between the court and agency staff regarding participant status and progress. Responsibilities
Attend policy meetings with members of the interdisciplinary team and ensure that changes in policy are implemented and communicated. Develop and maintain positive working relationships with members of the interdisciplinary team. Coordinate and facilitate multidisciplinary meetings; interact across systems efficiently and effectively. Support CCBHC Model of Care: integrated, trauma-informed, recovery-oriented and person-and-family-centered care; provide mental health, SUD and primary care screening services. Qualifications
Bachelor’s Degree in the social sciences or related field preferred. Case Management II certification “preferred” or obtain by taking the first available Case Management Certification course provided by ODMHSAS after hire. Strong commitment to the right and ability of each person served to live in normal community residences; access to supports and services. Skills to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues, with respect for client rights and preferences. Valid Oklahoma Driver License and satisfactory driving record; use personal automobile to travel to locations other than primary office. Strong organizational, oral and written communication, and presentation/training skills. Excellent program management skills and ability to conduct several projects simultaneously. Experience coordinating and facilitating multidisciplinary meetings and interacting across systems. CCBHC Model of Care
Family & Children’s Services uses a Certified Community Behavioral Health Clinic (CCBHC) model to provide integrated behavioral health care, with team-based and whole person care for adults and children with mental health and substance use challenges. Increased access to care and crisis services Expanded traditional community mental health and substance use services Care coordination and physical health screening for mental health clients Greater access to social services for clients’ economic and social needs Increased specialized services for veterans and those in need, including those affected by the opioid crisis Policies
