Responsibilities
Verify the Plan of Care prior to each visit and provide care according to delegation by the RN, physician’s orders, assessment data, and established standards and guidelines. Incorporate patient care goals from the Plan of Care into nursing visits, as evidenced by documentation in skilled nursing notes. Communicate significant findings or changes in condition to the RN Case Manager, physician, clinical supervisor, and other personnel involved in patient care. Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician as needed. Implement the plan for patient safety using patient, family, and community resources. Assist the patient in learning appropriate self-care techniques while delivering quality patient care. Suggest revisions to the plan of care in response to patient care issues and notify the RN Case Manager. Provide services requiring substantial and specialized nursing skills under the direction of a Registered Nurse or physician. Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions. Document medication regimens and update the medication profile according to established procedures. Treat patients and caregivers in a respectful and effective manner. Implement actions to prevent avoidable hospitalizations and ER visits. Provide positive, supportive communication to physicians, patients, families, visitors, and agency personnel. Qualifications / What is Required
Graduate of an accredited certificate, diploma, or associate degree program in nursing. Current state licensure as a Licensed Vocational/Practical Nurse. 1 year of experience in a clinical care setting; hospice experience preferred. Experience in a home health environment preferred. 50% travel required. Ability to sit, stand, bend, lift and move intermittently and lift 50–100 lbs. Valid driver’s license, valid insurance, and reliable transportation. Compensation
The compensation range is approximately $35 to $45 per hour, with potential additional earnings for work beyond quota, bonuses, on-call, or other performance-based compensation variables. Elara Caring determines compensation based on education, knowledge, skills, training, and experience. Compensation may vary by location. Reporting
