Nurse Practitioner - Palliative Care

Description We are seeking an experienced Nurse Practitioner to join our Palliative Care team located in MaineHealth’s Mountain Region. The goal of palliative care is to maximize quality of life and align the individual's medical care with what matters most to them. The Palliative Care NP will care for patients in both inpatient and outpatient settings, including hospitals and ambulatory clinics in Farmington, ME, Norway, ME, and North Conway, NH.
Responsibilities
Provide inpatient consultation services and ambulatory specialty palliative care in collaboration with the interdisciplinary team through expert assessment and treatment of complex symptoms, assisting with complex decision making, and providing psychosocial support.
Collaborate successfully as a member of the interdisciplinary team, taking responsibility for quality and continuity of care of assigned patients.
Embodies and serves as an ambassador for the philosophy of evidence-based care provision following NCP guidelines.
Collaborate with the patient’s primary care teams to facilitate identification of patient and family goals of care and access to appropriate support post‑discharge (e.g., home health, hospice, rehabilitation).
Complete timely and accurate documentation of all activities in the EPIC EMR system.
Participate in education of non‑palliative care providers and promote a culture transformation to reinforce primary palliative care education.
Respect patient confidentiality, privacy, and dignity.
Provide services in a timely and courteous manner with respect for the patient’s individual needs.
Participate in all practice group assignments and meetings.
Help educate medical students, residents, nurses, APP colleagues and other hospital staff about palliative care principles and practices.
Minimum Qualifications
Successful completion of an accredited Nurse Practitioner program, with a master’s degree level of education.
Current Board Certification as a Nurse Practitioner.
Active State of Maine and New Hampshire NP license or eligibility.
Current BLS Certification.
Previous APP experience required.
