Behavioral Health Provider
One Health formerly Community Health Partners is seeking a Wyoming‑licensed Behavioral Health Provider to join our clinical team in Sheridan, Wyoming. Eligible titles include Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage & Family Therapist (LMFT). Our multidisciplinary, team‑based model treats patients (including walk‑ins) for mental illness and substance abuse and provides preventive health and case management.
Responsibilities
Evaluate and treat patients within your licensed scope of practice.
Assess mental illness and substance abuse, and provide or refer to specialty treatment.
Develop and implement individualized treatment plans.
Provide preventive health/lifestyle advice and case management.
Offer consultation to other members of the care team.
Maintain accurate clinical documentation.
Qualifications
Master’s degree in Social Work (MSW), Counseling, or Marriage & Family Therapy and licensure as an LCSW, LPC, or LMFT in the State of Wyoming.
Experience with individual therapy and treatment planning.
BLS Certification upon hire.
Valid driver’s license.
Preferred Experience
Two (2) years of community health experience.
Two (2) years as a behavioral health provider.
Experience working with diverse cultures.

Compensation Base pay range: $81,935.00/yr – $92,599.00/yr (DOE).
