Case Manager -Domestic Violence Programs - NYC
Urban Resource Institute is seeking a Case Manager for Domestic Violence Programs in New York City. The role involves conducting intake assessments, engaging residents in treatment processes, coordinating housing and community-based referrals, and providing crisis response and case management support under clinical supervision.
Responsibilities
Conduct intake interviews, as assigned, to assess and evaluate residents for admission.
Engage residents in initial and ongoing treatment processes, through individual and group counseling, under clinical supervision of the Supervisor of Clinical Service or Program Director.
Develop and maintain housing referral resources; assist residents in locating and securing permanent housing, in conjunction with Housing Specialist.
Assist residents with referrals and placements in community-based resources including aftercare services.
Assess residents’ vocational and educational needs, aptitudes, and interests; develop and maintain placement and follow-up as appropriate after assessment and counseling.
Develop and maintain liaison activities with H.R.A. Public Assistance and Medicaid Administrators and Workers to assist eligible residents in securing Public Assistance and Medicaid by acting as a personal advocate.
Participate in training seminars to enhance clinical skills with the client population.
Attend weekly individual case supervision, group supervision, monthly case conferences, and staff meetings.
Function independently in times of facility coverage, including crisis situation actions per agency procedures.
Regularly assess the safety and well-being of clients; make appointments for residents’ physical exams.
Develop with the resident a contract of mutually agreed goals and objectives to be accomplished within an agreed timetable.
Respond to Hotline calls as appropriate.
Coordinate on-site services such as child care, child therapy, and recreation.
Identify concrete needs and provide support on an as-needed basis.
Perform other duties as assigned by the Social Work Supervisor.
Qualifications / Education
High school diploma.
Required Skills and Experience
Two (2) years of relevant training and experience, including experience working with victims of domestic violence; or Bachelor's degree in health or human services.
Two (2) years of crisis counseling experience preferred in a residential shelter for victims of domestic violence, with experience and demonstrated skills in group counseling.
Training and experience in assessing vocational and educational needs; familiarity with engaging clients and assisting eligible clients through obtaining Public Assistance, Medicaid and other benefits.
Knowledge of domestic violence issues; sensitivity to diverse client populations including LGBTQ+ and HIV-affected individuals, the elderly, and individuals with mental health issues.
Ability to deliver client services including strong communication and listening skills, documentation, and group counseling skills.
Computer literacy with knowledge of Microsoft Office Applications.
Flexibility in schedule as needed; excellent written and verbal skills.
Pay Rate: $23.07/hr - $25.82/hr
